Driver's Safety Course Completion Submittal Form

  1. Driver's Safety Certificate and DL Records Submittal
    This form allows you to submit your Driver's Safety Course Completion Certificate (Must Have Texas Department of License and Regulation Logo) and Texas 3A Certified Driving Record (Must Have Department of Public Safety Seal). In limited statutory instances the driving record may not be required. Refer to your Court Order. Allow 10-14 days for the court staff to process your submission. You may also check your case status online. If you are taking a Driver's Safety Course for a Deferral of Final Disposition, you may also use this form. It is the Defendant's responsibility to ensure all requirements of the order are satisfied and submitted properly.
  2. You must attach a scanned or PDF copy of your Driver's Safety Course Certificate of Completion bearing the TDLR logo SIGNED BY THE STUDENT and List Frisco Municipal Court on the Certificate or a variation thereof.
  3. You must attach a scanned or PDF copy of your certified DPS Texas 3A Driving Record if required.
